Constellation Award for Childhood Cancer

Every day in hospitals and clinics across the country, care team members work tirelessly to help families face the scariest and most stressful moments of their child’s brain cancer journey – providing comfort, compassion, and support in ways both big and small. The Pediatric Brain Tumor Foundation’s Constellation Award for Childhood Cancer offers families an opportunity to thank these incredible healthcare professionals for their hard work, dedication, and kindness.

If you would like to recognize a care team member for the positive difference they made in your family’s life, please fill out and submit the form below.

Guidelines for nominations:

  • Any parent/caregiver, pediatric brain tumor patient, survivor, or patient sibling can nominate a care team member. Please note: Nominations from children under the age of 18 must be submitted on their behalf by a parent/legal guardian.
  • Families can nominate any care team staff member, including administrative staff, medical professionals, members of the psycho-social care team, and other hospital or clinic staff.
  • Families may submit no more than one entry per quarter, and nominees may only win once per year.

After you submit a nomination, our family support team will follow up within one to three business days to confirm. Nominations are reviewed on a rolling basis by a team of Pediatric Brain Tumor Foundation board members and volunteers, who name three awardees each quarter.
